1. Consent

 

I, [Father's Name] of Selnar, do on this date declare that [Child's Former Name] is now a legal member of my household as my adopted child. I accept all the proper responsibilities and duties as [Child's Former Name]'s father.  

   

I, [Mother's Name] of Selnar, the Free Companion of [Father's Name], do on this date declare that [Child's Former Name] is now a legal member of my household as my adopted child. I accept all the proper responsibilities and duties as [Child's Former Name]'s mother. 

I, [Child's Former Name], do on this date declare that I accept the parental authority of [Father's Name] and [Mother's Name], and all other proper responsibilities and duties as their child.

2. Applicable Adjustments

[Child's Former Name] will now be known as [Child's New Name if Applicable].  

[Child's New Name if Applicable] will maintain her current caste of [CASTE].

[Child's Former Name] consents to allow [Father's Name] and [Mother's Name] to negotiate his/her future Free Companionships and all related material and/or financial arrangements as needed.
